These positions are located in the <strong>Bronx, and Brooklyn.</strong>&nbsp; <strong><em>Comunilife offers a generous benefits and paid time off package to all full time employees. &nbsp;The salary range for this position is up to $65,000 a year.</em></strong></span><span style=\"font-family: tahoma, sans-serif; font-size: 14px;\"><br><br><br></span><span style=\"color: rgb(0, 0, 0); background-color: rgb(255, 255, 255); font-size: 14px; font-family: tahoma, sans-serif;\"><strong>Essential Duties:</strong></span><span style=\"font-family: tahoma, sans-serif; font-size: 14px;\"><br></span><span style=\"color: rgb(0, 0, 0); background-color: rgb(255, 255, 255); font-size: 14px; font-family: tahoma, sans-serif;\">&bull; Ensure that the initial and medical assessments are conducted, and that comprehensive service plans are developed that identify mutually agreed upon long and short-term goals.</span><span style=\"font-family: tahoma, sans-serif; font-size: 14px;\"><br></span><span style=\"color: rgb(0, 0, 0); background-color: rgb(255, 255, 255); font-size: 14px; font-family: tahoma, sans-serif;\">&bull; Review and provide supervision in the development of case management service plans and ensure that reassessments are conducted on a quarterly basis (at minimum) to ensure that services are appropriate for the clients current needs.</span><span style=\"font-family: tahoma, sans-serif; font-size: 14px;\"><br></span><span style=\"color: rgb(0, 0, 0); background-color: rgb(255, 255, 255); font-size: 14px; font-family: tahoma, sans-serif;\">&bull; Provide oversight and supervision in the delivery of mental health and alcohol/substance abuse services and referrals, including assistance in accessing primary medical care, assistance with adherence to medication regimens, activities for daily living, nutritional services and delivery of any other services necessary for clients to increase their capacity for independent living.</span></p><p><span style=\"color: rgb(0, 0, 0);
