The <strong id=\"isPasted\">Director</strong> shall ensure all elements of Health Home care coordination focuses on achieving and setting New York State Department of Health (<strong><em>NYSDOH)</em></strong> measurable goals through quality-driven, cost-effective, culturally appropriate, person- and family-centered, care coordination. The Director shall ensure the efficient use of health information technology to ensure staffs link clients to needed services, are facilitating communication among clinical team members, to individuals and family caregivers, as well as to provide feedback as to practices, that are feasible and appropriate. The Director shall ensure the timely and comprehensive submission of billing information to the health home and coordinate with fiscal and others on billing and payment issues. &nbsp; The Director will further establish a continuous quality improvement program, and collect and report on data that permits an evaluation of increased coordination of care and chronic disease management on individual-level clinical outcomes, experience of care outcomes, and quality of care outcomes at the population level.&nbsp; display: inline !important; float: none;\">Oversight of daily program operations.</span></span></li><li>Complete and submit required reports to Lead Health Homes as noted in the Business Associate Agreements.</li><li>Review staff electronic daily activity logs to ensure effective time management, minimum required patients contacts are met and to identify potential concerns.&nbsp;</li><li>Ensure compliance with the NYSDOH Data Exchange Agreement on confidentiality.&nbsp;</li><li>Review and resolve all personnel related activities such as approving personal time, completing bi-weekly payroll, disciplinary and termination activities and ensuring compliance with Comunilife &nbsp;Policy &amp; Procedures.&nbsp;</li><li>Review and resolve all fiscal programmatic needs.\\</li><li>Ensure that all submissions to the health home for billing are timely and complete.</li><li>Work with fiscal to resolve any billing and/or payment issues.&nbsp;</li><li>Evaluate staff cohesion and resolve issues and concerns.</li><li>Ensure development and implementation of QI activities.</li><li>Ensure quality clinical services are performed within the spectrum of <strong style=\"background-color: transparent;\"><em>NYSDOH</em>&nbsp;</strong><span style=\"background-color: transparent;\">guidelines.</span></li><li>Attend Health Home Network Provider meetings as needed.</li><li>Provide support to clinical staff as needed.</li><li>Participate in the development and update of company&rsquo;s Health Home Policy &amp; Procedures.</li><li>Provide oversight in patient related interventions to ensure service provisions remain in a continuum.</li><li>Ensure all program staff upholds the agency&#39;s Code of Conduct/Code of Ethics and comply with all Federal, state and local law/regulation, including 42 CFR confidentiality and HIPAA privacy and security regulations.</li><li>Perform other duties as assigned by supervisor.</li></ul><p><strong style=\"background-color: transparent;\"><u>Education and Experience Requirement(s)</u></strong></p><ul><li><span style=\"background-color: transparent;\">Master&rsquo;s Degree in Human Services or related field, required.</span></li><li><span style=\"background-color: transparent;\">LMSW, preferred.</span></li><li>Must have at least 1-3 years&rsquo;
