Together, we keep getting better - advancing our mission to transform healthcare and serve as a leader of positive change.</p>\n<p> </p>\n<p>We invite you to listen to a message from our CNO who shares what it's like to be part of Team HMH at Jersey Shore University Medical Center:</p>\n<p> </p>\n<p></p>\n<h2>Responsibilities</h2>\n<p>A day in the life of a <strong>Clinical Coordinator</strong> at <em>Hackensack Meridian Health</em> includes:</p>\n<ul>\n <li>Answers telephones in accordance with established policies and procedures.  </li>\n <li>Utilizes professional tone and demeanor in all communication.</li>\n <li>Coordinates communication between sending and receiving facilities, and within the receiving facility.</li>\n <li>Accurately records details of all communication related to each transfer request in the electronic health record in an efficient and timely manner.</li>\n <li>Works in a collaborative manner with other members of the health care team.</li>\n <li>Utilizes clinical expertise, and collaborates with capacity management, nursing, physician/provider representatives, and other hospital departments to assure proper level of care and room assignment at the receiving facility.</li>\n <li>Maintains awareness of bed availability on all patient care units and outpatient departments involved in the inter-facility transfer and direct admission of patients.</li>\n <li>Demonstrates the ability to handle a high degree of pressure, heavy workloads, multiple requests, and numerous interruptions in a positive manner, establishing priorities for effective work completion.  Able to diffuse challenging situations and respond appropriately.</li>\n <li>Demonstrates expertise in working with multiple computer applications as well as all functions of the telephone system.</li>\n <li>Other duties as assigned.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Current and valid New Jersey Registered Nurse license is required</li>\n <li>BLS/CPR certification by American Heart Association is required</li>\n <li>At least 2 years of clinical experience in an acute care setting required</li>\n <li>Critical care and/or emergency nursing experience preferred</li>\n <li>Transfer center experience preferred</li>\n <li>BSN preferred</li>\n</ul>\n<p>If you feel that the above description speaks directly to your strengths and capabilities, then please apply today!</p>\n<h2>Starting Minimum Rate</h2>Minimum rate of $64,771.20 Annually\n<h2>Job Posting Disclosure</h2>HMH is committed to pay equity and transparency for our team members.
