Strong leadership and facilitation skills. Ability to work with all stakeholders involved in cases to support a positive experience and outcome on each case. Knowledge of local resources. Ability to develop creative discharge plans to support LOS of patients in the inpatient rehabilitation setting. Understanding of criteria and ability to apply for utilization management of cases.</p>\n<p><strong>Qualifications:</strong></p>\n<ul>\n <li>RN, MSW, LCSW, CRC, RN, CCM with Bachelor's degree as a minimum.</li>\n <li>1 year experience&nbsp;as a Case Manager or Social Worker in Acute Care or Inpatient Rehabilitation. May Consider a new grad with strong knowledge and skills.</li>\n <li>Experience working with patients and families that have acquired brain injuries or with Medically Complex Patient Populations that have chronic disabilities and multiple social and medical community resource needs.</li>\n <li>Strong communication skills to facilitate rehabilitation teams and families through complex discharge planning with the goal of community reentry – i.e. manage chaos.</li>\n <li>Individual must be accountable for the scope of their work and able to work independently to manage their caseload as well as with a team of 12 Case Managers to ensure the entire hospital receives needed Case Management services.</li>\n <li>Ability to delegate tasks to ensure efficiency in their day to day casework.</li>\n <li>Knowledge of or ability to research community resources both locally and in geographic areas beyond Jacksonville.</li>\n <li>Must be a team player and have strong leadership skills to facilitate discharges of complex patients back to their communities.</li>\n</ul>\n<p><strong>Desired Qualifications:&nbsp;</strong></p>\n<ul>\n <li>Complex Case Management, Discharge Planning, and Placement experience preferred.</li>\n <li>Experience with insurance and knowledge of Medicare and Medicaid benefits&nbsp;preferred.</li>\n <li>Demonstrate an ability to network and develop key relationships needed to support successful discharges.</li>\n</ul>\n<p><strong>Shift: </strong>PRN(work as needed)</p>\n<p><strong>Location:&nbsp;</strong>6209 Brooks Bartram Drive, Building 100, Jacksonville, FL 32258</p>",
