Our Values: Innovation, Accountability, Person-Centred, Health Promotion, Excellence, Sustainability, Accessibility Guelph Community Health Centre prioritizes people who face barriers to health and accessing services, and people who identify as: Homeless or at risk of homelessness Living on low income Newcomers to Canada with language barriers Having moderate to severe mental health and/or addiction issues Having moderate to severe disabilities Indigenous First Nations, Inuit and Métis Black, mixed race and racialized populations Experiencing intersectional oppression 2SLGBTQIA+ Vulnerable children and their families experiencing conflict, isolation or attachment struggles This Registered Practical Nurse (RPN) position will work at our clinics that offer HIV care, gender affirming care, and PrEP services in Kitchener, Guelph, and occasionally Cambridge. The RPN is a key member of an interprofessional team delivering trauma-informed, person-centred, and culturally safe care to priority populations. The role supports smooth patient flow at the clinic, performs client intake, assessments, and clinical procedures, provides thorough health education, and coordination of services and supplies across multiple locations. Clinical & Nursing Care Work as a member of the specialty HIV, gender affirming care, and PrEP clinic Practice at the full scope of RPN practice, consistent with CNO standards. Provide safe, high-quality nursing care in a fast-paced clinical environment. Complete nursing intake, vital signs, screening tools, and patient preparation for appointments. Administer medications, immunizations, and injections, in alignment with medical directives. Perform venipuncture, point-of-care testing, and collection of other specimens. Support sexual health care, including STI screening and treatment under medical directives. Care Coordination & Patient Flow Maintain smooth, timely movement of patients through appointments across multiple clinic sites. Triage clients and incoming messages to identify urgent needs. Support follow-up systems (lab follow-up, medication renewals, client calls). Provide patient education related to HIV care, PrEP, hormone therapy, and harm reduction. Clinic Operations Ensure adequate stock of medications, vaccines, medical supplies, and other required equipment. Follow infection prevention and control protocols, including cleaning and disinfection of clinical spaces. Support clinic setup and shutdown at different locations as scheduled. Utilize electronic medical records for accurate and timely charting, documentation, and communication. Required Qualifications: Current Registration as a Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO). Thorough knowledge and proficiency in nursing principles, practices, and CNO standards. 3–5 years broad-based practical nursing experience; community primary care preferred. Ability to provide compassionate, non-judgmental, trauma-informed care in challenging situations. Strong communication and teamwork skills and ability to work effectively within a multi-disciplinary environment. Ability to self-manage own time and prioritize tasks. Demonstrated commitment to community-based health care and harm reduction principles. Experience with electronic charting (i.e. Telus PS Suite) and computer proficiency (i.e. Microsoft Suite programs including Outlook, Word, and Basic Excel). G2 license and reliable access to a vehicle. Assets: Experience working with people living with HIV Experience working with folks who are part of transgender and gender-diverse communities Experience in sexual health, PrEP, hormone therapy, or crisis/mental health support Lived or living experience reflective of the communities we serve We know that experience comes in many forms.
