The role supports smooth patient flow at the clinic, performs client intake, assessments, and clinical procedures, provides thorough health education, and coordination of services and supplies across multiple locations.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Clinical &amp; Nursing Care</span></p>\n<ul>\n<li>Work as a member of the specialty HIV, gender affirming care, and PrEP clinic</li>\n<li>Practice at the full scope of RPN practice, consistent with CNO standards.</li>\n<li>Provide safe, high-quality nursing care in a fast-paced clinical environment.</li>\n<li>Complete nursing intake, vital signs, screening tools, and patient preparation for appointments.</li>\n<li>Administer medications, immunizations, and injections, in alignment with medical directives.</li>\n<li>Perform venipuncture, point-of-care testing, and collection of other specimens.</li>\n<li>Support sexual health care, including STI screening and treatment under medical directives.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Care Coordination &amp; Patient Flow</span></p>\n<ul>\n<li>Maintain smooth, timely movement of patients through appointments across multiple clinic sites.</li>\n<li>Triage clients and incoming messages to identify urgent needs.</li>\n<li>Support follow-up systems (lab follow-up, medication renewals, client calls).</li>\n<li>Provide patient education related to HIV care, PrEP, hormone therapy, and harm reduction.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Clinic Operations</span></p>\n<ul>\n<li>Ensure adequate stock of medications, vaccines, medical supplies, and other required equipment.</li>\n<li>Follow infection prevention and control protocols, including cleaning and disinfection of clinical spaces.</li>\n<li>Support clinic setup and shutdown at different locations as scheduled.</li>\n<li>Utilize electronic medical records for accurate and timely charting, documentation, and communication.<br></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Required Qualifications:</span></p>\n<ul>\n<li>Current Registration as a Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO).</li>\n<li>Thorough knowledge and proficiency in nursing principles, practices, and CNO standards.</li>\n<li>3–5 years broad-based practical nursing experience; community primary care preferred.</li>\n<li>Ability to provide compassionate, non-judgmental, trauma-informed care in challenging situations.</li>\n<li>Strong communication and teamwork skills and ability to work effectively within a multi-disciplinary environment.</li>\n<li>Ability to self-manage own time and prioritize tasks.</li>\n<li>Demonstrated commitment to community-based health care and harm reduction principles.</li>\n<li>Experience with electronic charting (i.e. Telus PS Suite) and computer proficiency (i.e. Microsoft Suite programs including Outlook, Word, and Basic Excel).</li>\n<li>G2 license and reliable access to a vehicle.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Assets:</span></p>\n<ul>\n<li>Experience working with people living with HIV</li>\n<li>Experience working with folks who are part of transgender and gender-diverse communities</li>\n<li>Experience in sexual health, PrEP, hormone therapy, or crisis/mental health support</li>\n<li>Lived or living experience reflective of the communities we serve<br></li>\n</ul>\n<p><br></p>\n<p>We know that experience comes in many forms.
