With three (3) conveniently located health centers, three (3) behavioral health sites and a friendly welcoming staff, PHC aspires to serve all the health care and wellness needs of the entire family.</p><p><strong>About the Role:</strong></p><p>The Social Worker plays a critical role in supporting individuals and families facing a variety of challenges by providing comprehensive social services and mental health support. This position involves assessing client needs, developing and implementing care plans, and coordinating with healthcare and community resources to promote well-being and stability. The Social Worker will engage in crisis intervention to address urgent situations, ensuring client safety and access to necessary services. Through empathetic counseling and case management, the role aims to empower clients to overcome obstacles and improve their quality of life.
