This position is responsible for coordinating and managing comprehensive hospice care plans that address the physical, emotional, and psychosocial needs of patients and their families. The RN Case Manager serves as a liaison between patients, families, interdisciplinary team members, and external healthcare providers to ensure seamless communication and continuity of care. By conducting thorough assessments and regularly evaluating patient progress, the RN Case Manager ensures that care goals are met while respecting patient dignity and preferences. Ultimately, this role contributes to enhancing the quality of life for patients during their end-of-life journey through expert clinical management and empathetic support.</p><p><strong>Minimum Qualifications:</strong></p><ul><li>Current and valid Registered Nurse (RN) license in the state of Georgia.</li><li>Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or related fields.</li><li>Strong knowledge of hospice care principles, symptom management, and end-of-life care practices.</li><li>Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.</li><li>Ability to manage multiple cases simultaneously while maintaining attention to detail and documentation accuracy.</li></ul><p><strong>Preferred Qualifications:</strong></p><ul><li>Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.</li><li>Experience with electronic medical records (EMR) systems and hospice-specific documentation software.</li><li>Background in case management or care coordination within a hospice or home health setting.</li><li>Familiarity with Medicare hospice regulations and reimbursement processes.</li><li>Demonstrated skills in patient and family education, grief counseling, and cultural competency.</li></ul><p><strong>Responsibilities:</strong></p><ul><li>Conduct comprehensive patient assessments to develop individualized hospice care plans.</li><li>Coordinate interdisciplinary team meetings to review and update patient care goals.</li><li>Monitor patient conditions regularly and adjust care plans as necessary to meet evolving needs.</li><li>Provide education and support to patients and their families regarding disease progression, symptom management, and available resources.</li><li>Serve as the primary point of contact for patients, families, and healthcare providers to facilitate effective communication and care coordination.</li><li>Ensure compliance with hospice regulations, documentation standards, and quality assurance protocols.</li><li>Advocate for patient rights and preferences while promoting comfort and dignity in end-of-life care.</li><li>Collaborate with community resources and support services to enhance patient and family support.</li></ul><p><strong>Skills:</strong></p><p>The RN Case Manager Hospice utilizes clinical nursing skills daily to assess patient conditions, manage symptoms, and implement care plans tailored to individual needs.
