Description

NURSE PRACTITIONER SUMMARY : As part of a collaborative agreement with a physician as mandated by Ohio law and under the direction of the AxessPointe Community Health Center, Inc. (API) Medical Director, the NP/PA will provide quality primary medical and behavioral health services to adults, children and families in a holistic manner within the northeast Ohio area. SUPERVISOR: Medical Director or Designee DUTIES AND RESPONSIBILITIES : Examines, diagnoses, and treats illnesses and injuries of patients throughout the lifespan. Provides well-child, well-woman and well-man health maintenance exams to new and established patients. Orders appropriate screening and diagnostic lab and radiographic exams. Interprets results of physical exams and screening/diagnostic tests to implement appropriate care plans for the patient. Prescribes medicines, therapeutic agents and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority. Supports health promotion, health education, health empowerment and disease prevention interventions for all patients. Provides age-appropriate care to all patients. Promotes patient and family centered care through a collaborative team approach. Participates in established call group as necessary. Refers patients to specialty practices as appropriate and needed. Provides medical care to patient as part of mutual patient care with specialist. Confers with collaborating physician about patient care as necessary. Documents and maintains patient records of services provided according to program standards. May instruct medical students or nursing students. Operates within a scope of practice as agreed to by the FNP/PA, the collaborating physician, the medical director and in compliance with Ohio state law. Supervision and Administrative experience helpful You may be asked to perform any of the job duties within your respected scope of practice at any AxessPointe location.
