    "description": "<h2>Overview</h2>\n<p>AltaPointe’s Adult Residential Services are designed for consumers with serious and persistent mental illness who require social rehabilitation and community support. Combining medical, nursing and clinical services, our staff provides care and treatment for more than 200 adults in need of intensive service provision. The Adult Residential Services Program consists of group homes, which are supervised 24 hours/day when consumers are on the premises.  Expected outcomes are the same for each adult group home: to prevent re-hospitalization and to maximize and maintain successful community placement. Residential care focuses on the rehabilitation and return to the community of seriously mentally ill consumers.</p>\n<h2>Responsibilities</h2>\n<p><strong>Primary Job Functions:</strong></p>\n<p> </p>\n<p>General Responsibilities:</p>\n<ul>\n <li>Transports patients to and from appointments and outings</li>\n <li>Effectively collaborates with ARS/Outpatient Offices and outside agencies regarding consumer appointments and follow-up care</li>\n <li>Assists in retrieval of medications from community resources</li>\n <li>Delivers and retrieves needed items from AltaPointe and community resources</li>\n <li>Assists patients with personal care issues, when needed and as appropriate</li>\n <li>Demonstrate the ability to recognize the elements of a crisis state and takes a proactive approach to deescalate or resolve the situation</li>\n <li>Ongoing compliance with Nurse Delegation Program, including recertification requirements</li>\n <li>Models appropriate behavior including communication skills, grooming, manners, etc.</li>\n <li>Complies with routine health and safety tasks for vehicles including but not limited to operating vehicle lifts and assisting patients with limited mobility.</li>\n <li>Meets daily productivity standards through individual and group BLS services when working in the home.</li>\n</ul>\n<p> </p>\n<p>Supervision and Consultation:</p>\n<ul>\n <li>Seeks supervision and consultation as needed</li>\n <li>Accepts and employs suggestions for improvement</li>\n <li>Attends regularly scheduled staff meetings</li>\n <li>Demonstrates the willingness to work at any ARS facility based on staffing needs</li>\n</ul>\n<p> </p>\n<p>Clinical Record Keeping:</p>\n<ul>\n <li>Documents in a timely fashion per AltaPointe policy</li>\n <li>Documents in a legible manner, types of consumer services on a progress note</li>\n <li>Documents detailed BLS services provided, individualized consumer responses, and progress toward goals </li>\n</ul>\n<p> </p>\n<p>Courteous and respectful attitudes towards patients, visitors, co-workers, and outside medical personnel:</p>\n<ul>\n <li>Treats patients with care, dignity and compassion</li>\n <li>Respects patient’s privacy and confidentiality</li>\n <li>Is pleasant and cooperative with others</li>\n <li>Assists patients, visitors, and personnel as needed</li>\n <li>Personal values don’t inhibit ability to relate and care for others</li>\n <li>Is sensitive to the patient’s needs, expectations and individual differences</li>\n <li>Is gentle and calm with patients, families, and outside medical providers</li>\n</ul>\n<p> </p>\n<p>Administrative and Other Related Duties as Assigned:</p>\n<ul>\n <li>Actively participates in Performance Improvement activities</li>\n <li>Actively participates in AltaPointe committees as required</li>\n <li>Completes assigned tasks in a timely manner</li>\n <li>Follows AltaPointe policies and procedures</li>\n <li>Attends appropriate in-services training and other workshops</li>\n <li>Any other duties that are assigned by supervisor or designee</li>\n</ul>\n<p><strong>Physical Requirements of the Job:</strong></p>\n<p> </p>\n<ul>\n <li>There is frequent to constant standing while organizing/stocking office supplies</li>\n <li>Sitting is frequent by nature; however, extended sitting could be up to 3 hours in physician waiting room)Walking various distances on grounds can be frequent and the furthest distance would be up to 300 feet</li>\n <li>Squatting/lunging frequently to assist patients into car or van.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Minimum Qualifications:</strong>  </p>\n<p>High school diploma or equivalent.  Has and maintains a valid Alabama driver’s license. Must be 23 years of age or older and must have proof of current liability insurance.</p>",
