    "ExternalDescriptionStr": "<p>Primary Accountability</p>\n<p>Responsible for the provision of primary care medical services in a Patient-Centered Health Home context, with a focus on access to care.</p>\n<p>Description of Primary Responsibilities:&nbsp;</p>\n<ol>\n <li>Responsible for the direct provision of primary care medical services, meeting all established productivity expectations.\n  <ol>\n   <li>Will provide examination and will review appropriate laboratory, referral and imaging&nbsp;results to determine a diagnosis.</li>\n   <li>Expected to document visits accurately utilizing appropriate templates and record relevant data to attain a diagnosis.</li>\n   <li>Accountable for attaining budgeted visits.</li>\n   <li>Accountable for meeting clinical care measures.</li>\n   <li>Accountable for locking records in the 72-hour window.</li>\n  </ol></li>\n <li>Responsible for meeting Advanced Practice Provider professional focus and goals.\n  <ol>\n   <li>Provide comprehensive, continuous, and coordinated medical care for acute and chronic illness at all ages within the context of a family unit in a community setting.</li>\n   <li>Comprehensive health care including health promotion and maintenance, prevention of illness and disability and application of holistic healthcare model with focus on addressing both protective factors and risk factors affecting the health and wellbeing of patient.</li>\n   <li>Management of chronic diseases and acute illness with a broad range of knowledge in procedural skills, coordination of care with specialists, and utilization of community resources when appropriate.</li>\n   <li>Advocate access to health care with a focus on structured evaluation, early intervention, and health promotion.</li>\n   <li>Consult with supervising physician or other team members on cases that need further input.</li>\n   <li>Works within and helps to promote the Patient-Centered Health Home model of care.&nbsp;</li>\n  </ol></li>\n <li>Accountable for providing excellent customer service and care.&nbsp;\n  <ol>\n   <li>Consistently behaves courteously when interacting with patients/ family members, support staff, and Network staff.</li>\n   <li>Provider remains aware of wait time during the daily workflow.</li>\n   <li>Responsible for providing the patient with educational materials and resources appropriate to the patient’s health literacy needs.</li>\n  </ol></li>\n <li>Accountable for patient status as measured by relevant clinical quality measures.\n  <ol>\n   <li>Promotes and participates in daily huddles.&nbsp;</li>\n   <li>Incorporates the participation of ancillary health team members in managing the care of patients/families.</li>\n   <li>Responsible for setting and monitoring self-management plans.</li>\n   <li>Responsible for assisting in the assurance of clinical procedures and the maintenance of up-to-date clinical protocols.</li>\n   <li>Completes chart and peer review responsibility in a timely manner.</li>\n  </ol></li>\n <li>Assists the patient care team with performance improvement efforts, staff education, and FHCN promotion.\n  <ol>\n   <li>Responsible for assisting the patient care team with performance evaluations and quality improvement.</li>\n   <li>Where relevant participate in rotation of medical, physician assistant students and dental residents.</li>\n   <li>Participates in Network Health Fairs and other community events.&nbsp;</li>\n   <li>Participates in site and provider meetings in a meaningful way.</li>\n  </ol></li>\n <li>Meets Network and credentialing expectations for licensure and employment.</li>\n <li>Performs other duties as assigned.</li>\n</ol>\n<p>Description of Primary Attributes</p>\n<p>Professional &amp; Technical Knowledge:</p>\n<ol>\n <li>Performance of the duties and responsibilities of the job requires the equivalent of extensive formal training as a Nurse Practitioner or Physician Assistant, including an understanding of the application of the theory and practices of the profession to the operation of the organization as part of a master’s degree (or other applicable extended training program).</li>\n</ol>\n<p>Technical Skills:</p>\n<ol>\n <li>Ability to create highly complex documents in Microsoft Word, including linking multiple files and embedding objects linked to other documents.&nbsp;</li>\n <li>Ability to use advanced functions of Microsoft Excel, such as to create and manage databases, including creating standardized reports, or link multiple worksheets and workbooks.</li>\n <li>Ability to develop sophisticated presentations in Microsoft PowerPoint, including the use of embedded objects, transitions, and other elements.</li>\n</ol>\n<p>Licenses &amp; Certifications: Licensed in the state of California as a Nurse Practitioner and/or licensed in the state of California as a Physician Assistant.&nbsp;</p>\n<p>Communications Skills:</p>\n<ol>\n <li>Requires employees to effectively communicate their professional opinions and extrapolations of information they collect, synthesize/analyze.</li>\n <li>Employees must determine appropriate methods of communicating information through the use of tables, graphs, charts, and other visual forms.&nbsp;</li>\n <li>Duties require the preparation and execution of presentations to large groups.</li>\n</ol>\n<p>Physical Demands: The physical demands described here in this job description represent those that must be met by an employee to perform the essential functions of this position successfully. The employee is frequently required to stand or walk and occasionally lift and/or move up to 20 pounds.</p>\n<p>$142,853.66 - $228,565.86 + PLUS&nbsp;SIGNIFICANT ADDITIONAL ANNUAL EARNING POTENTIAL&nbsp;through outpatient productivity and quality pay, generous retention payments, shift differential pay, and extra shift pay. &nbsp;Sign-on bonus/relocation assistance, annual CME days and reimbursement also available. &nbsp;Generous health and wellness benefits and retirement packages also offered.</p>",
