Will monitor Members&rsquo; health concerns, help with collaboration-of-care, provide appropriate screenings and referral to PCP, and provide education on wellness topics and self-care and collaborate with the Team to lead the Member community according to our Mission and Core Values. &nbsp;</span></p><p><span style=\"font-family: georgia, serif; font-size: 16px;\"><strong>Specific Responsibilities:&nbsp;</strong></span></p><ul><li style=\"font-family: georgia, serif; font-size: 16px;\">Provide health and wellness services for assigned members&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Provide&nbsp;assistance&nbsp;per psychiatrist&rsquo;s directives with medication issues and monitoring</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Provide appropriate screenings and referrals to PCP&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Assist psychiatrist with members&rsquo; collaboration-of-care&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Provide groups, workshops, and engaging programming educating members on health and wellness topics&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Teach members health management skills and support increasing independence in this area</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Complete all documentation on day of service delivery&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Assist with monitoring medication documentation for accuracy&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Participate in case consultation as requested by Director or Clinicians&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Conduct and document collaborative activity with various providers, referral sources, insurance companies, and regulatory agencies&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Participate in inter-agency meetings when needed&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Complete trainings as required for regulatory compliance&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Participate in daily Morning Team Meetings and weekly Team Meeting&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">Participate in Annual Performance Evaluations&nbsp;</li></ul><p><span style=\"font-family: georgia, serif; font-size: 16px;\"><strong>Required Performance Expectations&nbsp;</strong>&nbsp;</span></p><ul><li style=\"font-family: georgia, serif; font-size: 16px;\">LPN will participate in a multidisciplinary treatment team and will support&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">and implement interventions and clinical directives as directed by the Psychiatrist the Clinician or the Team.&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">LPN will demonstrate in their work with Members at ALL times, compassion, and concern when supporting a Member through embracing our organization&#39;s Mission and Core Values.&nbsp;</li><li style=\"font-family: georgia, serif; font-size: 16px;\">LPN will approach Member engagement from a non-judgmental stance understanding that a Member&#39;s behavior is driven by&nbsp;past experience, which may include trauma.</li><li style=\"font-family: georgia, serif; font-size: 16px;\">LPN will treat and speak to Members with supportive kindness even when a Member demonstrates intense behavioral or emotional actions.
