    "description": "<h2>Overview</h2>\n<p><strong>Patient Care Navigator</strong></p>\n<p> </p>\n<p>Samaritan Daytop Health</p>\n<p>$24.72/ hour</p>\n<p>Mon-Fri 9AM-5PM</p>\n<p> </p>\n<p> </p>\n<p> </p>\n<p>The <strong>Patient Care Navigator </strong>plans, organizes, coordinates, monitors and evaluates the provision of services at Samaritan Daytop Health's Clinic. They collaborate with the team in promoting quality patient care to the communities we serve through the appropriate utilization of resources and coordination of professional and ancillary staff.  Additionally, the Patient Care Navigator will provide direct patient care if needed. </p>\n<h2>Responsibilities</h2>\n<p> </p>\n<ul>\n <li>Conducts agency visits to client's providers.</li>\n <li>Maintains a caseload as assigned by the Care Coordination Supervisor.</li>\n <li>Utilizes approved Health Home assessment tools and Health Information Technology (HIT) to complete initial and annual assessments and to develop an appropriate care plan of service needs.</li>\n <li>Completes an accurate monthly HML assessment for each assigned member.</li>\n <li>Provides referrals, develops linkages and follows up on client services including timely documentation in all Electronic Medical Records for corresponding Health Homes.</li>\n <li>Uses Health Information Technology (HIT) dashboards to link services and communicate among care management team, providers, members and their families/caregivers.</li>\n <li>Assumes role of patient advocate and conforms to policies and regulations governing patient rights, confidentiality and the HIPAA Privacy Rule.</li>\n <li>Maintains a clean, safe and therapeutic work environment. </li>\n <li>Identifies, investigates and reports patient/environmental safety factors and makes recommendations for improvement.</li>\n</ul>\n<h2>Qualifications</h2>\n<p> </p>\n<ul>\n <li>Associates Degree in Human Services, Sociology, Psychology or a related field.</li>\n <li>Minimum Three (3) years supervisory experience strongly preferred and/or three years progressive nursing experience, preferably in an outpatient setting.</li>\n <li>Must be creative, intuitive team player with excellent communication, organizational and interpersonal skills.  </li>\n <li>Computer literacy including proficiency in Microsoft Office Suite and EMR.</li>\n <li>Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.</li>\n</ul>\n<p><strong>Additional Information</strong></p>\n<p>This is a non-exempt role with a rate of $24.72 per hour. This role also includes a full benefits package. </p>",
