Your primary objective will be to establish permanency for these children, ensuring they find loving and stable homes where they can thrive.</p>\n\n<p><strong>Essential Duties and Responsibilities:</strong></p>\n\n<ul>\n\t<li>Provide comprehensive case management services to children and families within the foster and adoptive care system.</li>\n\t<li>Develop and formulate permanency goals and concurrent plans tailored to the unique needs of assigned children.</li>\n\t<li>Conduct assessments of child abuse/neglect risks and ongoing safety of children.</li>\n\t<li>Assess child and family functioning, strengths, needs, stressors, and coping mechanisms to tailor support.</li>\n\t<li>Research and refer children and families to necessary services to meet their specific needs.</li>\n\t<li>Evaluate the effectiveness and impact of provided services, ensuring constant improvement.</li>\n\t<li>Advocate for assigned children in court proceedings, providing crucial testimony on their behalf.</li>\n\t<li>Offer parent education and support to strengthen family dynamics.</li>\n\t<li>Participate in an on-call rotation to address urgent needs and emergencies.</li>\n\t<li>Ensure all required documentation standards are meticulously met and maintained.</li>\n\t<li>Conduct face-to-face meetings with assigned children and out-of-home care providers according to prescribed time frames.</li>\n\t<li>Schedule, coordinate, and facilitate Family Support Team (FST) meetings within the prescribed time frames.</li>\n\t<li>Secure appropriate and least restrictive out-of-home placements for assigned children, ensuring their safety and well-being.</li>\n\t<li>Arrange for medical, dental, and routine health services for children in out-of-home placements.</li>\n\t<li>Assist families in meeting the cost of care expenses, alleviating financial burdens.</li>\n\t<li>Responsible for being trauma aware, sensitive, and responsive to youth, families and colleagues through the completion of new hire and annual trauma informed education and training.<br />\n\t&nbsp;</li>\n</ul>\n\n<p><strong>Why Work with Us?</strong></p>\n\n<p>Every Child&#39;s Hope is committed to creating a supportive and nurturing environment for both its staff and the families it serves.
