Occupational Therapist (OT) – Elderwood at North Creek
Location: Elderwood at North Creek
Compensation : $44.01 - $66.02 / hour

Position Overview
Occupational Therapy staff members assist with ensuring the health and well-being of residents by providing comprehensive, quality therapy services. This position is responsible for administering skilled occupational therapy treatments while ensuring that the highest degree of quality care is maintained at all times.
About Elderwood at North Creek
Elderwood at North Creek is an 82-bed skilled nursing and rehabilitation facility located in a scenic, woodland setting near the Hudson River in the North Country. The facility provides a full continuum of care including skilled nursing, long-term care, subacute rehabilitation, and specialized therapy services.
Our interdisciplinary team delivers personalized care in a warm, community-focused environment. With a bright, spacious rehab gym and state-of-the-art equipment, we offer physical, occupational, and speech therapy for both short-term and long-term residents.
Responsibilities
Evaluate and develop individualized treatment plans for residents
Administer skilled occupational therapy treatments
Monitor and document resident progress and outcomes
Collaborate with physicians, nursing staff, and interdisciplinary team members
Educate residents and families on therapeutic techniques and care plans
Ensure compliance with all regulatory and facility standards
Qualifications
Current New York State Occupational Therapist License (required)
Strong clinical, communication, and documentation skills
Ability to work collaboratively in a team-oriented environment
Experience in skilled nursing or rehab settings preferred, but not required

Responsibilities
Occupational Therapist:
Initiates skilled occupational therapy treatments in accordance with physician’s orders; maintains required contact with physician.
Maintains written evaluations, progress notes, MDS records and discharge summaries for individuals in occupational therapy program.
Confers with other health care staff regarding special needs of indviduals to enhance rehabilitation or maintenance of function.
Provides input for Care Planning meetings and attends during absences of the Director of Occupational Therapy.
Maintains statistical records of indviduals’ treatments.
Assists with in-service training programs upon request.
Records and updates individual’s care plan.
Submits requisition for supplies needed in the treatment of individuals to Director of Occupational Therapy.
Assumes all duties of the Director of Occupational Therapy, if applicable, during absences including supervision of COTA’s and Occupational Therapy Aides. Assumes responsibility for all necessary documentation.

Qualifications
Occupational Therapist:
Graduation from an approved school of occupational therapy required.
State license to practice occupational therapy required.
Two years’ experience in occupational therapy preferred.
Interest in occupational therapy treatment programs for the geriatric and subacute populations utilizing a team approach.
Ability to direct others necessary.
Ability to communicate well verbally and in writing required.
Current, valid CPR/BLS credential
Sense of responsibility and ability to work well with residents, families, and other staff members required.
Qualities of maturity, diplomacy, and ability to work well with others required.
Ability to work with geriatric and chronically ill residents, as well as other professionals in a friendly and helpful manner necessary.
Ability to perform tasks to established standards of excellence required.
This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
