Hours assigned by supervisor; on call responsibilities. With advanced qualifications or significant experience, this role will provide specialized or high-level support, while maintaining quality, service excellence and ethical standards.</p>\n<p> </p>\n<p><strong>Pay: $40.00/hour</strong></p>\n<p><strong>Direct Support Professionals are provided a paid New Employee Orientation and receive training on CPR/First Aid, Safety Care, Defensive Driving, and Medication Administration Program (MAP). Seven Hills Foundation is an equal-opportunity employer committed to diversity and inclusion. </em><em><strong>If you experience any difficulty in applying or need assistance, please contact the main office at 508-755-2340.</strong></em></p>\n<h2>Responsibilities</h2>\n<ul>\n <li><p>Assess and coordinate healthcare services for individuals, ensuring timely follow-up and communication with relevant clinical and residential staff</p></li>\n <li><p>Accompany and assist individuals with medical appointments in accordance with DDS guidelines</p></li>\n <li><p>Document all nursing interventions, health updates, and medication changes in Electronic Medical Records (EMR) and communication logs</p></li>\n <li><p>Monitor and maintain current medication and treatment orders, report side effects or adverse reactions as appropriate</p></li>\n <li><p>Review and support implementation of Individual Service Plans (ISPs), including self-medication, behavior plans, hygiene, and therapy programs</p></li>\n <li><p>Train residential staff on medication administration and health protocols</p></li>\n <li><p>Ensure up-to-date and compliant health records in line with DDS standards</p></li>\n <li><p>Communicate medical recommendations clearly to all program staff involved in the individual's care</p></li>\n <li><p>Maintain safety and monitor for health-related risks within the program</p></li>\n <li><p>Participate in staff meetings, mandatory trainings, and clinical supervision with the Health Care Director or program RN</p></li>\n <li><p>Perform medical interventions and additional responsibilities as assigned</p></li>\n <li><p>Uphold confidentiality, dignity, and respect for all individuals receiving services</p></li>\n <li><p>Foster a collaborative, supportive, and professional environment in alignment with Seven Hills Foundation values</p></li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education & Experience:</strong></p>\n<ul>\n <li>High school diploma, GED, or HiSET required.</li>\n <li>Licensed practical nurse required (LPN)</li>\n <li>1-3 years of related experience preferred.</li>\n <li>CPR/First Aid and MAP certification required (paid training provided if needed).</li>\n</ul>\n<h3><strong>Skills and Knowledge:</strong></h3>\n<ul>\n <li>Strong communication and organizational abilities.</li>\n <li>Compassionate and patient approach to care.</li>\n <li>Commitment to fostering inclusion and independence.</li>\n <li>Ability to perform physical tasks, including lifting up to 50 pounds (or 200 pounds with assistance), and remain alert during shifts.</li>\n</ul>",
