Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital.2. Accompany each resident on all medical visits. Stay with resident(s) until visit is complete.3. Observe the following guidelines at each visit:     a. Sign in resident upon entrance.     b. Present resident’s documents to office personnel.4. Before leaving doctor’s office, please observe the following:     a. Obtain resident’s documents.     b. Obtain any new prescriptions.     c. Check to make sure doctor has written on the doctor’s order form.     d. If follow-up appointment necessary, obtain appointment cards.5. After returning to facility, report to RCM and be prepared to answer questions concerning visit.6. Performs other duties as required and assigned.</p>\n<h2>Qualifications</h2>\n<li> Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle.</li>\n<li>Possess a valid state driver’s license.</li>\n<li>Qualified as a Certified Nurse Assistant (C.N.A.).</li>\n<li>Clean driving record.</li>\n<li>Clean criminal record.</li>\n<li>Any other state required certifications</li>\n<p> </p>\n<p>Diversicare is committed to being an equal opportunity employer.
