Certified Nursing Assistant (CNA) / Medication Aide Elderhaus, Inc. Full-Time, In-Person Elderhaus PACE is seeking a compassionate, dependable, and participant-centered CNA / Medication Aide to provide hands-on care, medication support, and daily assistance to older adults in our PACE program. This role is ideal for someone who enjoys building meaningful relationships, supporting older adults with dignity and respect, and working as part of a collaborative interdisciplinary care team. Position Summary Under the supervision of the Clinical Supervisor, RN, the CNA / Medication Aide provides medication administration, direct participant care, and support with activities of daily living. This position helps participants reach and maintain their highest level of functioning while following physician orders, care plans, organizational policies, industry standards, and applicable federal and state regulations. The CNA / Medication Aide assists with restorative and supportive care, activity program implementation, participant monitoring, documentation, and maintaining a safe and respectful environment. This role also functions as a member of the interdisciplinary team and supports the overall care and well-being of Elderhaus PACE participants. Key Responsibilities The CNA / Medication Aide will: Provide care according to Elderhaus PACE approved care plans, nursing orders, policies, procedures, and standards. Perform procedures outlined in the Nursing Procedure Manual and maintain working knowledge of rules and regulations related to CNA II services. Deliver quality care with full attention to participant rights, dignity, respect, confidentiality, and decision-making. Assist participants with activities of daily living, including bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care, toileting, and personal hygiene. Assist participants with walking, wheelchair use, self-exercise, transfers, and safe movement. Assist with lifting and transporting participants using proper lifting techniques. Monitor and assist participants with meals and nourishments and record consumption as needed. Provide bed pan, urinal, and incontinent care. Assist with ostomy care as assigned. Insert urinary catheters following sterile technique, empty foley catheters, record amounts, and collect specimens as needed within scope and training. Change participant clinic bed linen and maintain clean participant care areas. Transport participants to activities and support engagement in the activity program. Record participant weight, measurements, vital signs, food intake, output, and other assigned observations. Document care, participant activities, appearance, and relevant health information according to Elderhaus PACE policy. Notify the supervisor of participant changes, concerns, difficulties, errors, or occurrences. Note changes in a participant’s physical or emotional health and communicate this information to the appropriate staff person. Attend and participate in regularly scheduled meetings to discuss participant care and progress. Participate in interdisciplinary team activities as needed and carry out the plan of care as directed. Perform duties ordered by a licensed nurse, including oropharyngeal suctioning, gastrostomy feedings, and room set-up and oxygen flow monitoring, as assigned and within scope. Once trained by a registered nurse, check fingerstick blood glucose using aseptic technique, sanitize the glucometer, accurately record readings, and promptly report abnormal findings. Check wound dressing placement as ordered and report soiled or non-intact dressings to the nurse. Answer call lights promptly for all participants, regardless of assignment. Provide and maintain a safe environment for participants. Support quality improvement initiatives. Follow risk management, safety, infection control, employee TB screening, security, fire safety, OSHA, and other applicable requirements. Handle potentially infectious specimens with appropriate biohazard precautions and practice Universal Precautions. Maintain confidentiality of participant, client, family, and organizational information. Maintain participant medical records and fulfill Elderhaus PACE charting and reporting requirements. Participate in continuing education classes, required staff meetings, and training. Perform other duties as assigned. Medication Administration Responsibilities The CNA / Medication Aide will also: Demonstrate understanding of basic medication administration information and medical terminology. Administer medications using appropriate technique for dosage, form, route, and accurate amount. Assist with medication administration in accordance with physician orders. Maintain proper records in the Medication Administration Record and participant medical record. Coordinate the processing of new orders, re-orders, and discontinued orders between the participant’s physician, pharmacy, and community. Use special medication administration or monitoring techniques as indicated and within scope. Monitor participants for changes in condition and follow appropriate steps when health status changes occur. Qualifications Qualified candidates should have: High school diploma or equivalent. Current Certified Nursing Assistant certification. Current Certified Nursing Assistant II certification. Successful completion of a Certified Medication Technician or Medication Aide program with current certification. Current CPR certification. At least one year of experience working with frail or elderly populations required. At least three months of work experience in basic nursing procedures for nursing assistants. Valid driver’s license, proof of insurance, and reliable transportation. Medical clearance for communicable diseases and required immunizations prior to direct participant contact. Ability to complete required position-specific competencies before assuming participant care responsibilities. Skills and Abilities The successful candidate will demonstrate: Compassionate, respectful, and participant-centered care. Strong attention to detail and accurate documentation. Good communication, active listening, and interpersonal skills. Ability to build effective relationships with participants, caregivers, families, and staff. Ability to work collaboratively as part of an interdisciplinary team. Work Environment and Physical Requirements This position works in a participant care environment serving older adults with complex needs. The role may involve direct personal care, participant transfers, mobility assistance, exposure to infection, and caring for individuals with various cognitive abilities. The position requires the ability to communicate verbally with staff, participants, caregivers, families, and community partners. The role also requires standing, walking, occasional pushing, pulling, and lifting.
