Will provide services such as assessment, service planning, advocacy, linking and coordinating and teaching life skills, to assist families in attaining an independent and self-sufficient lifestyle. All services are to adhere to the fidelity of the Wraparound Model, with a strong emphasis on culturally competent service delivery and practice.</span></span></p>\n\n<p>&#160;</p>\n\n<p><span style=\"font-size:12pt;\"><strong><u><span style=\"font-size:11pt;\">EDUCATION AND EXPERIENCE:</span></u></strong></span></p>\n\n<ul>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">Bachelor&#8217;s Degree in human services or a related field and meet the qualifications of a child mental health professional as defined by the Michigan Department of Community Health.</span></span></li>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">A current State of Michigan Social Work registration preferred.</span></span></li>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">A minimum of 3 years supervised experience in the examination, evaluation, and treatment of minors and their families.</span></span></li>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">Experience using the Wraparound Model</span></span></li>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">Current, valid driver's license.</span></span></li>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">Facilitators must complete the MDCH 3 Day Wraparound Training within the first 3 months of hire.</span></span></li>\n</ul>\n\n<p>&#160;</p>\n\n<p><span style=\"font-size:12pt;\"><strong><u><span style=\"font-size:11pt;\">DUTIES AND RESPONSIBILITIES:</span></u></strong></span></p>\n\n<ul>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">The Facilitator will assist each child, youth, and family in defining the frequency of child and family team meetings, the standard for meeting frequency should occur at least weekly until the family begins to transition toward graduation. Once the transition/graduation plan is developed, the meetings should then occur at least bi-weekly for no longer than 3 months.</span></span></li>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">The Facilitator must work with the family to uncover youths/family&#8217;s strengths and needs and provide assistance with any immediate needs and complete the initial strengths assessment and needs and priorities within thirty (30) days of enrollment.</span></span></li>\n\t<li><span style=\"font-size:12pt;\"><span style=\"font-size:11pt;\">The Facilitator must work with the family and submit a comprehensive Support Plan (proactive and reactive crisis/safety plan) - within 1 week of enrollment.
