Your work will focus on empowering clients through hands-on support, guidance, and evidence-based skill-building strategies.</p>\n\n<h3><strong>What You&rsquo;ll Do</strong></h3>\n\n<ul>\n\t<li>\n\t<p>Provide one-on-one skills training focused on daily living, self-care, and social interaction</p>\n\t</li>\n\t<li>\n\t<p>Identify each client&rsquo;s unique needs and develop individualized training plans</p>\n\t</li>\n\t<li>\n\t<p>Support clients in achieving greater independence and community engagement</p>\n\t</li>\n\t<li>\n\t<p>Collaborate with families, caregivers, and external partners (including court systems and social services)</p>\n\t</li>\n\t<li>\n\t<p>Educate clients and families on mental health, substance use, and available resources</p>\n\t</li>\n\t<li>\n\t<p>Assist clients in accessing community services and participating in activities</p>\n\t</li>\n\t<li>\n\t<p>Maintain timely and accurate documentation in the electronic medical record (EMR) system</p>\n\t</li>\n\t<li>\n\t<p>Work closely with treatment teams, providers, and referral sources to ensure coordinated care</p>\n\t</li>\n\t<li>\n\t<p>Uphold all organizational policies, ethical standards, and confidentiality requirements</p>\n\t</li>\n</ul>\n\n<p><br />\nTo support a safe and healthy environment for the individuals we serve and for one another, all employees of Radiant Health are required to comply with organizational health and infection prevention requirements as a condition of employment. This includes providing documentation of immunization status, completing required health screenings, and adhering to communicable disease prevention protocols applicable to the role and work environment.
