Join us in making a real difference in the health of our community.</em></p>\n<h2>Job Summary</h2>\n<p>The Limited Permit Provider works in collaboration with the primary care provider and all members of the care team. Develops care initiatives to assess and coordinate chronic disease care management for their patient population in a manner that is medically appropriate and cost effective. Responsible and accountable for direct and indirect patient care for designated patient populations. Assesses healthcare needs and identifies community resources. Serves as patient advocate and assists in identification and improvement of service delivery.</p>\n<h2>Responsibilities</h2>\n<p>All below responsibilities are similar to that of a fully licensed nursing home physician, and a Limited Permit Provider. All applicants are required to fulfill the below responsibilities under the general supervision of fully licensed providers.</p>\n<ul>\n <li>Completes comprehensive evaluation of all new patients.</li>\n</ul>\n<ul>\n <li>Effectively and proactively manages chronic medical conditions to maximize functional and medical recovery.</li>\n</ul>\n<ul>\n <li>Rapidly identifies and addresses new medical conditions that may impair functional and medical recovery.</li>\n</ul>\n<ul>\n <li>Meets with interdisciplinary team for each patient at least weekly and documents progress towards goals, barriers to goals, plans to address barriers, and current discharge plan.</li>\n</ul>\n<ul>\n <li>Attends scheduled patient and family meetings as necessary to ensure effective communication and discharge planning.</li>\n</ul>\n<ul>\n <li>Ensures that patient goals are considered in rehabilitation planning, goal setting, and discharge planning.</li>\n</ul>\n<ul>\n <li>Effectively works with patients, families, and interdisciplinary teams to meet established goals.</li>\n</ul>\n<ul>\n <li>Rapidly identifies when clinical course is not progressing towards goals and effectively intervenes to correct barriers, communicate with team and patient, and work with team and patient to establish new goals as needed.</li>\n</ul>\n<ul>\n <li>Ensures seamless care transitions by direct physician to physician communication for community, facility, or hospital discharges.</li>\n</ul>\n<ul>\n <li>Effectively participates in Post-Acute and facility performance improvement activities. • Completes scheduled “on call” work requirements.</li>\n</ul>\n<ul>\n <li>Ensures that all medical care follows accepted principles of Geriatric and Palliative care.</li>\n</ul>\n<ul>\n <li>Complies with all applicable Federal, State and local laws, regulations, and requirements as well as facility/agency specific policies and procedures.</li>\n</ul>\n<ul>\n <li>Works toward defined clinical metrics as determined by the medical department.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Qualifications:</strong></p>\n<ul>\n <li>Medical Graduate (International or National) Passed portions of the United States Medical Licensing Examination (USMLE)</li>\n</ul>\n<ul>\n <li>Educational Commission for Foreign Medical Graduate <strong>(ECFMG)</strong></li>\n</ul>\n<ul>\n <li>Possess/ eligible for active Limited Permit for Medicine according to NYS Office of Professions.</li>\n</ul>\n<ul>\n <li>Excellent oral and written communication skills.</li>\n</ul>\n<ul>\n <li>Excellent bedside manners and organizational skills.</li>\n</ul>\n<ul>\n <li>Clinical Knowledge</li>\n</ul>\n<ul>\n <li>Must be comfortable with EMR navigation (Sigmacare, PointClickCare, Wellsky, Visual, etc.)</li>\n</ul>\n<p>Salary: $50,000-$70,000</p>\n<h2>Equal Opportunity Employer</h2>\n<p><em><strong>Equal Opportunity Employer Essen Health care is proud to be an equal opportunity employer, and we seek candidates who desire to work in and serve an ethnically diverse population.</strong></em></p>",
