The Hospice Aide provides direct support and personal care for hospice patients, to assist in their day-to-day needs, provide comfort and enhance their quality of life. The Hospice Aide is responsible for following patients&rsquo; individual Plan of Care, providing in-person care, maintaining documentation and coordinating with the patient&rsquo;s RN Case Manager.</p>\r\n\r\n<p><strong>Essential Functions and Skills</strong></p>\r\n\r\n<ul>\r\n\t<li>Provide personal care, including bathing, dressing, oral hygiene, and bed linen changes.</li>\r\n\t<li>Assist patients with mobility, transfers, and exercises as instructed by nursing staff.</li>\r\n\t<li>Offer assistance with meals, feeding, and light household tasks as assigned.</li>\r\n\t<li>Monitor and report patient condition changes to the RN Case Manager.</li>\r\n\t<li>Administer routine skin care to prevent breakdown and ensure patient comfort.</li>\r\n\t<li>Support medication reminders per state regulations.</li>\r\n\t<li>Maintain a clean, safe, and organized patient environment.</li>\r\n\t<li>Provide emotional support and respite for family caregivers.</li>\r\n\t<li>Use equipment safely and appropriately for patient care needs.</li>\r\n\t<li>Adhere to all documentation and HIPAA compliance standards.</li>\r\n\t<li>Participate in quality assessment and performance improvement activities.</li>\r\n\t<li>Perform other duties as assigned.</li>\r\n</ul>\r\n",
