Our purpose and passion are to empower patients, their families, and our communities by helping individuals achieve recovery and optimal wellness of the mind, body, and spirit.</p>\n<h2>Responsibilities</h2>\n<p><strong><u>Job Summary</u></strong></p>\n<p>Duties to include but not limited to: the assessment/evaluation of patient withdrawal followed by the execution of appropriate medication detox protocols; administration of prescribed medications and patient education. </p>\n<p> </p>\n<p><strong><u>Duties and Responsibilities: <em>to include but not limited to the following</em>:  </u></strong></p>\n<p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.</p>\n<p>The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. </p>\n<ul>\n <li>Provide patients with a safe medically managed detox by assessing signs and symptoms of withdrawal. Monitors the effects of medications administered and documents accurately and appropriately.</li>\n <li>Reports and documents medication errors, pharmacy errors, packaging errors, medication count concerns, etc.</li>\n <li>Ensures that ongoing assessments are conducted as determined by the patient’s condition using COWS & CIWAS and re-prioritizes care accordingly.</li>\n <li>Maintains neat and accurate narcotic control inventories and all documentation related to the administration of medications.</li>\n <li>Promotes patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, answers patient questions.</li>\n <li>Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations.</li>\n <li>Resolves patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques.</li>\n <li>Documents patient care services by charting in EMR.</li>\n <li>Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs.</li>\n <li>Contributes to team effort by accomplishing related results as needed.</li>\n <li>Participates in the onboarding of new nurses for medication administration.</li>\n <li>At end of shift communicate to incoming LPN pertinent data regarding patient activity and conducts medication count.</li>\n <li>Treats patients, and colleagues with dignity and respect; honors each person’s right to privacy/maintains very strict patient and colleague confidentiality; respects all individual differences; supports cultural diversity.</li>\n <li>Knows role and responds appropriately to all codes related to emergency situations.</li>\n <li>Consistently arrives to work on time and adheres to attendance policies.</li>\n <li>Other duties, as needed.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Education/Experience:</strong></p>\n<ul>\n <li>Current LPN License in State of operations.</li>\n <li>LPN Diploma & State License Required</li>\n <li>Knowledge of drug and alcohol treatment preferred.</li>\n <li>Working knowledge of Google Products</li>\n</ul>\n<p> </p>\n<p><strong><u>Physical Requirements</u></strong></p>\n<p>AAC is committed to principles of equal opportunities for all employees. Actual compensation within this range will depend on the candidate’s skills, experience, education, and relevant qualifications.
