Our team is committed to partnering with our seniors to remain at home through promoting open communication and shared decision making while providing excellent care and services.</p>\n<p> </p>\n<p><strong>Schedule </strong></p>\n<ul>\n <li>Monday - Friday, 8a - 430p</li>\n <li>No Weekends</li>\n <li>No Evenings</li>\n <li>No Holidays</li>\n <li>No On Call</li>\n</ul>\n<p><strong>Benefits </strong></p>\n<ul>\n <li>Competitive Pay</li>\n <li>Comprehensive Benefits</li>\n <li>401k</li>\n <li>PTO</li>\n <li>Paid Holidays Off</li>\n</ul>\n<p><strong>Requirements of the Financial Eligibility and Reimbursement Coordinator</strong></p>\n<ul>\n <li>Bachelor’s degree with applicable experience in Health related Field (Preferred) </li>\n <li>Must have a driver's license, reliable transportation, and car insurance</li>\n <li>Knowledge of Medicare/Medicaid regulations (Required)</li>\n</ul>\n<p> </p>\n<p><strong>Responsibilities:</strong></p>\n<ul>\n <li><p>Responsible for coordination of Medicaid (MA) and Medicare benefits with participants, families, and governing agencies. </p></li>\n <li>Maintain current electronic case management records, including annual MA recertifications, Skilled Nursing Facility day 31 submissions, and invoicing.</li>\n <li>Complete documentation of all visits (including episodic, routine, phone calls, home visits, etc.) against the corresponding care plan elements.</li>\n <li>Communicate participant changes to team members.</li>\n <li>Provide administrative assistance (applications, forms etc.)/community resources to participants as needed.</li>\n <li>Assist participants in successful completion of the medical assistance recertification application including submission of all relevant documentation within required timeframes to maintain financial eligibility for Senior LIFE.</li>\n <li>Keep the participant and family updated about the participant’s MA eligibility, Medicare application stats, and any changes with the participant’s financial status.<p> </p></li>\n</ul>\n<p>EOE</p>",
