To further this mission, Callen-Lorde promotes health education and wellness, and advocates for LGBTQ+ health issues. About the Role: Do you have a passion for supporting the health and well-being of others? Are you committed to social justice and ensuring access to quality healthcare for all? If so, Callen-Lorde wants to hear from you! We are seeking a Patient Navigator to join our dedicated team and play a vital role in supporting the needs of our patients living with HIV. As a Patient Navigator, your responsibilities will include: Act as a patient advocate, ensuring patients' biopsychosocial needs are identified, addressed, and connected to appropriate services. Provide field-based case management and adherence support services to HIV-positive patients. Work collaboratively with care coordinators, medical providers, and behavioral health professionals to facilitate patient retention and adherence to primary medical care. Assist patients in navigating the healthcare system, overcoming barriers to care, and connecting them with essential resources. Advocate for patients' rights and ensure they receive culturally competent care. Document patient interactions and progress notes to ensure continuity of care. Contribute to a team-oriented environment, fostering a supportive and inclusive space for patients. Work Schedule: This is a full-time position with evening hours. Strong customer service orientation with an understanding of patient-led treatment. Experience working with individuals with a spectrum of chronic illness conditions preferred. Desired Characteristics, Skills Ability to handle multiple, simultaneous tasks. Ability to work independently yet be team focused. Ability to remain calm, focused, and helpful under stress. Motivation to provide excellent patient care. Excellent time management skills and attention to detail. Commitment to the mission of Callen-Lorde Community Health Center.
