Case Management</strong></h4><p>&bull; Conduct outreach in collaboration with community partners to identify individuals in need of services<br>&bull; Connect persons served to medical care, insurance, and social, mental health, and financial supports<br>&bull; Perform ongoing assessments and develop individualized care plans<br>&bull; Provide referrals and resource navigation based on client needs<br>&bull; Conduct home visits and transport clients to appointments (medical, legal, social services, etc.)<br>&bull; Provide health education, interpretation, and assistance accessing services<br>&bull; Maintain accurate documentation and data entry in CareWare or other EHR systems</p><h4><strong>Community Engagement &amp; Prevention</strong></h4><p>&bull; Participate in community meetings focused on health access and education<br>&bull; Collaborate with HIV Services team on prevention strategies (HIV/HCV/STI, overdose prevention)<br>&bull; Screen, assess, and counsel individuals at risk for HIV/HCV/STI or opioid overdose<br>&bull; Collect specimens for testing (venous blood draw, finger stick, oral swab, urine specimen)<br>&bull; Stay current on HIV, STI, Hepatitis, substance use, and harm reduction best practices through trainings</p><h4><strong>Program Support &amp; Operations</strong></h4><p>&bull; Assist with operations of the BAMSI Community Food Pantry (inventory, ordering, pickup, distribution)<br>&bull; Build and maintain professional relationships with clients, staff, and community partners<br>&bull; Follow all agency policies, procedures, and safety protocols, including universal precautions<br>&bull; Maintain flexibility to work occasional evenings and weekends as needed<br>&bull; Perform additional duties as assigned</p><hr><h3><strong>What You Bring:</strong></h3><h4><strong>Education &amp; Experience:</strong></h4><p>&bull; Associate or Bachelor&rsquo;s degree in a human services field&nbsp;<strong>preferred</strong>, OR<br>&bull; Minimum of 2 years professional or volunteer experience in a related field <strong>required</strong><br>&bull; Case management or care coordination experience <strong>required</strong></p><h4><strong>Preferred Experience:</strong></h4><p>&bull; Experience working with individuals living with HIV/AIDS<br>&bull; Experience working with substance use/addiction populations<br>&bull; Familiarity with harm reduction principles</p><h4><strong>Skills &amp; Qualifications:</strong></h4><p>&bull; Strong communication, organizational, and multitasking skills<br>&bull; Ability to work independently and collaboratively in a team environment<br>&bull; Cultural competency and comfort working with diverse populations<br>&bull; Bilingual abilities (Haitian Creole, Cape Verdean Creole, Portuguese, Spanish) <strong>strongly preferred</strong><br>&bull; Excellent documentation and computer skills (Microsoft Word, Teams, Outlook)</p><h4><strong>Additional Requirements:</strong></h4><p>&bull; Phlebotomy skills&nbsp;<strong>preferred</strong> or willingness to learn<br>&bull;
