We specialize in the care of high-acuity older adults with multiple chronic illnesses, including Alzheimer’s disease and other forms of dementia.</p>\n<p><br></p>\n<p>Our team is committed to delivering comprehensive, integrated care in a warm, home-like environment—emphasizing effective communication, compassionate service, and exceptional satisfaction for both residents and their families.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">POSITION SUMMARY</span></p>\n<p>The Registered Nurse (RN) – Weekend Day RN (On-Call) plays a vital role in ensuring timely, high-quality hospice care during weekends, evenings, and holidays. This position provides responsive clinical support and symptom management, helping maintain continuity and excellence of care across all settings.</p>\n<p><br></p>\n<p>This role is ideal for an experienced, independent hospice nurse who is passionate about providing compassionate, patient-centered care and can confidently manage on-call responsibilities.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">PRIMARY RESPONSIBILITIES</span></p>\n<ul>\n<li>Respond promptly to patient and family calls, triaging needs and providing compassionate, timely hospice care.</li>\n<li>Perform comprehensive nursing assessments including physical, emotional, and psychosocial evaluations.</li>\n<li>Assess, manage, and alleviate symptoms such as pain, shortness of breath, agitation, nausea, anxiety, and other distressing conditions.</li>\n<li>Implement and adjust the plan of care in collaboration with the hospice physician and interdisciplinary team to meet changing patient needs.</li>\n<li>Administer medications and perform prescribed treatments according to hospice protocols and physician orders.</li>\n<li>Conduct medication reconciliation at each visit to ensure safety, accuracy, and appropriateness.</li>\n<li>Provide education and guidance to families and caregivers on symptom management, medication administration, and end-of-life comfort measures.</li>\n<li>Offer emotional and spiritual support to patients and families, maintaining sensitivity to cultural and personal values.</li>\n<li>Coordinate with the interdisciplinary team (IDG), including social workers, chaplains, CNAs, and volunteers, to ensure seamless and holistic care.</li>\n<li>Report and communicate changes in condition promptly to the hospice physician and leadership team.</li>\n<li>Participate in on-call triage, facility coordination, and home visits as needed to maintain continuous care coverage.</li>\n<li>Ensure that all clinical notes, assessments, interventions, medication updates, and communications are accurately documented in the EMR within 24 hours of the patient encounter.</li>\n<li>Attend and actively contribute to IDG meetings, staff trainings, and performance improvement initiatives.</li>\n<li>Uphold all regulatory, compliance, and safety standards in accordance with state and federal hospice guidelines.</li>\n</ul>\n<p><br><br></p>\n<p><span style=\"font-weight: bold\">QUALIFICATIONS</span></p>\n<ul>\n<li><span style=\"font-weight: bold\">Current Registered Nurse (RN) license</span> in the State of California (required).</li>\n<li>Minimum 1 year of hospice or palliative care experience (preferred).</li>\n<li>CHPN certification preferred but not required.</li>\n<li>Strong clinical assessment and critical thinking skills; able to make autonomous decisions in the field.</li>\n<li>Excellent communication and empathy when supporting patients and families through end-of-life care.</li>\n<li>Reliable transportation, valid driver’s license, and proof of auto insurance (required).</li>\n<li>Proficiency with EMR systems and electronic documentation.</li>\n<li>Bilingual abilities in Spanish, Hmong, or other languages commonly spoken by our patient population are a plus.</li>\n</ul>\n<p><br><br></p>\n<p><span style=\"font-weight: bold\">WORK SCHEDULE</span></p>\n<ul>\n<li><span style=\"font-weight: bold\">6:00 AM – 10:00 AM:</span> In-person facility visits (on-site)</li>\n<li><span style=\"font-weight: bold\">10:00 AM – 10:30 AM:</span> Unpaid meal break</li>\n<li><span style=\"font-weight: bold\">10:30 AM – 2:30 PM:</span> Documentation and paperwork (office or home)</li>\n<li><span style=\"font-weight: bold\">2:30 PM – 5:00 PM:</span> On-call availability and response as needed <em>(clock in only if called)</em></li>\n<li><span style=\"font-weight: bold\">Compensation:</span> Guaranteed 8 hours/day, plus additional pay for any calls between 2:30 PM – 5:00 PM</li>\n</ul>\n<p><br><br></p>\n<p><span style=\"font-weight: bold\">WORK ENVIRONMENT &amp; BENEFITS</span></p>\n<ul>\n<li>Work performed in both an office and residential care setting, engaging directly with residents, families, and staff.</li>\n<li>Comprehensive benefits package including:\n<ul>\n<li>Competitive wages</li>\n<li>Health, Dental, and Vision insurance</li>\n<li>Paid Sick Leave</li>\n<li>401(k) with Deferred Profit-Sharing Plan</li>\n</ul>\n</li>\n<li>Opportunities for ongoing professional growth and career development within a supportive, mission-driven organization.</li>\n</ul>",
