Location: Rockland, NY (mobile)

Salary Range: $50,000 - $55,000 annually

Summary Description:
The Social Care Navigator works independently or as part of a larger team to provide screenings, navigation of services, and case management for eligible Medicaid enrollees as they relate to their social determinants of health needs. The Social Care Navigator will screen Medicaid members for unmet health-related social needs. If members qualify for any additional services, the Social Care Navigator is responsible for navigating & connecting members with such resources. Once connected, the Social Care navigator will communicate with the member to ensure closed loop referrals have been completed.
Responsibilities:
Provide comprehensive Social Care Navigator services, including screening & navigation services to eligible Medicaid Members via phone, in-person, & virtually. Document within the Unite Us electronic health record Develop and maintain an in-depth knowledge and understanding of the range of services (including eligibility criteria) available in the Social Care Network and existing local social services infrastructure. Participate in all required SCN trainings and meetings Develop care plans for clients based on eligibility for services and identification of needs. Communicate with community agencies and service providers. Follow established agency policies and maintain professional standards. Perform other duties as assigned. Competencies:
Effective outreach and engagement skills. Familiarity with community resources. Strong time management and organizational skills. Ability to work independently and efficiently. Proficiency in using Electronic Health Record systems. Practice in a recovery-oriented, person-centered methodology. Key Performance Indicators (KPIs):
Meets productivity expectations for clients screened and navigated as per contract metrics Establishes successful linkages to community resources. Maintains timely and accurate documentation. Completes and maintains all required trainings under the SCN model. Positive client outcomes and satisfaction. Effective collaboration with community partners. Requirements:
Associate’s degree in a relevant field or equivalent. E.g. social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or other human services field. Basic computer knowledge: typing, communication tools, scheduling tools and web browsers. Proficiency in the Microsoft Office suite and database management systems. Fluency in English (Reading, Writing, Speaking). Valid driver’s license. Must obtain and maintain Defensive Driver Certification within 30 days of hire. Must be and maintain clearance by applicable regulatory clearances as required. Legally eligible to work in the United States without sponsorship.
Special Considerations :
The Social Care Navigator will be using agency vehicles and is responsible for adhering to the Agency Driver Policy. Work hours, reporting structure, and on-site location are subject to change based on program needs.

This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter. You can be held liable by both the civil and criminal legal systems for intentionally failing to make a report.
