In this role, you’ll work directly with individuals to assess skills, identify support needs, and help create meaningful pathways toward employment and independence.</span></p><p><span style=\"font-size:14px;\">This position is a great fit for someone who enjoys building relationships, and making a direct impact in the community. Every day looks a little different, from meeting with participants and families to visiting job sites, conducting assessments, and collaborating with local employers.<br><br><strong>Pay Rate:</strong> $23.25/hr<br><strong>Schedule: </strong>FT<strong> </strong>40 Hrs. Monday - Friday days and evenings depending on client availability</span></p><h3><strong><span style=\"font-size:14px;\">What You’ll Do:</span></strong></h3><ul><li><span style=\"font-size:14px;\">Conduct vocational assessments, interviews, and evaluations to help determine employment goals and job compatibility</span></li><li><span style=\"font-size:14px;\">Meet with participants, families, referral sources, and support teams to gather information and build individualized employment recommendations</span></li><li><span style=\"font-size:14px;\">Coordinate caseloads, documentation, and individual plans through the electronic documentation system</span></li><li><span style=\"font-size:14px;\">Visit community job sites and employment partners to assess opportunities and participant success</span></li><li><span style=\"font-size:14px;\">Observe and document participant skills, accommodations, and support needs in real-world work environments</span></li><li><span style=\"font-size:14px;\">Develop and maintain positive relationships with local employers and community partners</span></li><li><span style=\"font-size:14px;\">Attend intake and pre-service meetings for new participants</span></li><li><span style=\"font-size:14px;\">Ensure compliance with CARF, DVR, DMH, DDRB, and agency standards</span></li><li><span style=\"font-size:14px;\">Maintain billable hours and complete all required trainings</span></li></ul><h3><strong><span style=\"font-size:14px;\">What We’re Looking For:</span></strong></h3><ul><li><span style=\"font-size:14px;\">High school diploma or GED required; Bachelor’s degree in social services or related field preferred</span></li><li><span style=\"font-size:14px;\">Experience working with individuals with intellectual and/or developmental disabilities preferred</span></li><li><span style=\"font-size:14px;\">Strong communication, organization, and problem-solving skills</span></li><li><span style=\"font-size:14px;\">Ability to manage multiple caseloads and priorities effectively</span></li><li><span style=\"font-size:14px;\">Comfortable working independently and as part of a team</span></li><li><span style=\"font-size:14px;\">Experience with Microsoft Office programs including Outlook, Word, and Excel</span></li><li><span style=\"font-size:14px;\">Valid driver’s license required</span></li></ul><h3><span style=\"font-size:14px;\">Why Join Community Living?</span></h3><p><span style=\"font-size:14px;\">At Community Living, Inc., we believe everyone deserves the opportunity to thrive in their community.
