In collaboration with the Health and Wellness Director, this role provides high-quality care through health assessments, medication management, and coordination with residents, families, and the care team. Responsibilities include monitoring resident health, addressing changes in condition, staff training and supervision, and promoting a safe and supportive environment that aligns with our mission to provide exceptional care. KEY RESPONSIBILITIES Perform resident assessments and reassessments according to policies, monitoring any changes in condition. Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services. When necessary, participate in hands-on care, medication administration, and specific nursing tasks. Schedule and assign caregiving and med tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances. Supervise, train, and support care team members, promoting teamwork and adherence to care standards. Assist in the transcription of physicians' orders and the review of new physicians' orders. Maintain accurate and up-to-date resident records, including care plans, incident reports, and medical documentation. Coordinate care planning with home health agencies on-site, working within community and company policies. Address residents’ special needs, requests, and complaints. Coordinate admissions with the sales team, including supervising move-ins. Understand the community's care regulations along with state-specific regulations. Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
