To provide initial emotional, spiritual, psychosocial assessments, ongoing counseling, bereavement services, and community education, outreach, and referral. The Hospice social worker is an integral part of the Hospice IDG. Qualifications Additionally, the Hospice Social Worker will: Performs initial psychosocial, emotional, spiritual, and bereavement assessments and assists in the development and implementation of goal directed IDG care plan. Conducts ongoing reassessments of patient/family needs and counseling as required. Provides short term crisis intervention and individual or family counseling when indicated. Provides services, under the direction of a physician (who approves the plan of care). Participates as a member of the Bereavement Team as assigned. Observes, assesses, and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family. Assumes the active role of advocate for the patient/family unit. Develops and maintains contact with appropriate community agencies and services to promote interagency cooperation and to facilitate related referrals. Documents comprehensive psychosocial, emotional, and spiritual assessment clearly and concisely on time. All patient/family visits, telephone contacts, and referral actions are recorded in the patient record per policy. Provides ongoing counseling related to issues of death and dying to the patient and family as needed. Attends staff meetings, IDG, and other meetings as assigned and appropriate. Participates in the orientation program as assigned. Adheres to all Hospice policies. Assumes responsibility for own personal and professional development and maintenance of skills in social work. Exhibits Hospice philosophy in all job-related roles. The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patients' home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit.

    "qualifications": "<p><span style=\"font-family:Aptos,sans-serif\">Additionally, the <strong>Hospice</strong> <strong>Social Worker</strong> will: </span></p>\n\n<ul style=\"list-style-type:square\">\n\t<li><span style=\"font-family:Aptos,sans-serif\">Performs initial psychosocial, emotional, spiritual, and bereavement assessments and assists in the development and implementation of goal directed IDG care plan.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Conducts ongoing reassessments of patient/family needs and counseling as required. </span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Provides short term crisis intervention and individual or family counseling when indicated. </span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Provides services, under the direction of a physician (who approves the plan of care).</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Participates as a member of the Bereavement Team as assigned.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Observes, assesses, and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Assumes the active role of advocate for the patient/family unit.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Develops and maintains contact with appropriate community agencies and services to promote interagency cooperation and to facilitate related referrals.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Documents comprehensive psychosocial, emotional, and spiritual assessment clearly and concisely on time.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">All patient/family visits, telephone contacts, and referral actions are recorded in the patient record per policy.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Provides ongoing counseling related to issues of death and dying to the patient and family as needed.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Attends staff meetings, IDG, and other meetings as assigned and appropriate.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Participates in the orientation program as assigned.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Adheres to all Hospice policies.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Assumes responsibility for own personal and professional development and maintenance of skills in social work.</span></li>\n\t<li><span style=\"font-family:Aptos,sans-serif\">Exhibits Hospice philosophy in all job-related roles.</span></li>\n</ul>\n\n<p><span style=\"font-family:&quot;Aptos&quot;,sans-serif\">The clinical record shall contain notes for each service provided. A clinical note will be completed for each visit made to a patients&#39; home (including a private residence, assisted living facility, group homes, or skilled nursing facility) within 48 hours of the visit. </span></p>\n",
