Work with our unique program and team of healthcare professionals, which offer a complete range of health-related services, all designed to keep participants living in their own community or home as long as possible. The PACE CNY model is a Managed Long-Term Care program which provides an alternative to nursing home placement.  The goal is to maximize the independence of the elderly and enrich their quality of life by optimizing their functional ability. This is achieved with the various services that are available to our PACE participants, including our very own PACE transportation system, 2 day centers that provide medical clinics and regularly scheduled primary care appointments, dining services, and social/recreational activities.  </p>\n<p> </p>\n<p>We offer a competitive wage, mileage reimbursement, tuition reimbursement, a free employee health center, health/dental insurance, and opportunities for career and personal growth.</p>\n<h2>Responsibilities</h2>\n<p>Top Five Responsibilities:</p>\n<ul>\n <li>Assist participants with Activities of Daily Living by providing direct personal care and homemaker services to participants at home and/or at the Day Center</li>\n <li>Promote optimal health by assisting in the development, provision and revision of a participants Plan of Care</li>\n <li>Assist with meals by serving and feeding participants and cleaning up after meal completion</li>\n <li>Monitor participants' health by noting changes in physical or emotional health and communicating this information to the Nurse and Interdisciplinary Team</li>\n <li>Follow Plan of Care by recording, monitoring and assisting (retrieving and opening container only) the participant with self-administered medications</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>Must have own reliable vehicle with proof of active NY State driver's license</li>\n <li>Current NYS certification as a Home Health Aide or willingness to be trained to become certified</li>\n <li>Ability to read, write and speak the English language</li>\n <li>Successful completion of physical capability requirements</li>\n</ul>\n<p>INDCERT</p>\n<h2>Benefits/Compensation</h2>\n<ul>\n <li>Pay Range $21.52 - $23.64 / hour</li>\n <li>Excellent medical, dental, prescription and vision insurance</li>\n <li>Disability coverage</li>\n <li>Tuition Reimbursement Program</li>\n <li>401(k) Retirement/Pension Plans</li>\n <li>Discounted Gym Membership</li>\n <li>Paid Time-Off Accrual</li>\n <li>Fun events for employees</li>\n <li>Convenient locations with free parking</li>\n <li>Employee Coaches - to help you achieve life goals</li>\n <li>Diaper Assistance Program</li>\n <li>Opportunity for career growth and movement within 19 sites</li>\n</ul>",
