Brief Summary of purpose:
The Director, PACE Nursing Services provides strategic leadership, operational oversight, and clinical guidance for all nursing functions across Fallon Health’s PACE centers. This role ensures that nursing practice across Primary Nursing, Home Care Nursing, Clinic Nursing, and Transitions of Care reflects the highest standards of quality, safety, and participant-centered care. The Director leads program-wide nursing initiatives, oversees policy development, evaluates clinic operations, and supports clinical teams to optimize nursing workflows, standardize nursing care, and uphold PACE regulatory compliance.
This position is a key member of the leadership team and partners closely with the program leaders to drive excellence in nursing care delivery throughout PACE.
Responsibilities
Nursing leadership and program oversight:
Provide leadership for all nursing services across PACE centers, ensuring consistent practice standards, clinical excellence, and adherence to PACE regulatory requirements.
Serve as the primary nursing leader responsible for the vision, strategy, and implementation of nursing initiatives across PACE
Collaborate with PACE leaders to align nursing services with organizational goals
Provide leadership and guidance to nursing staff, fostering a positive and collaborative work environment.
Serve as a clinical guide, resource, and mentor to nursing leaders across the program.
Conduct routine site visits to each PACE center to assess nursing workflows, nurse training and education needs, and quality performance.
Evaluate clinic flow and operational efficiency; identify gaps and implement improvements to optimize participant experience and clinical outcomes.
Partner with Site Directors, nursing leaders and others to ensure nursing practices support standardized, safe, timely, and coordinated care.
When applicable, engage external partners to support clinical needs
Chair and lead the PACE Nursing Committee, establishing organizational nursing priorities, standardizing practice, and supporting shared governance.
Facilitate program-wide communication, professional development, and alignment among nursing leaders and frontline nursing staff.
Promote a culture of collaboration, accountability, and continuous learning.
Provide mentorship, clinical guidance, and coaching to Primary Nursing, Home Care Nursing, Clinic Nursing, and specialty nursing teams across PACE
Supervise and support the Transitions of Care manager and the Transitions of Care nurses, ensuring effective coordination of care across inpatient and outpatient settings,
Supervise and support personnel responsible for PACE staff development and education
Partner with HR and site leadership to support nursing recruitment, retention, competency assessment, succession planning, and corrective actions when needed.
Regulatory and quality governance
Develop, implement, and ensure compliance with PACE nursing policies and procedures.
Ensure nursing standards are evidence-based, and follow federal, state, and company regulations.
In collaboration with the director of Quality & Regulatory Compliance, oversee nursing compliance and quality improvement efforts across the program
In coordination with PACE and nursing leaders, support efforts to meet utilization metrics, patient satisfaction, and MLR goals.
Ensure nursing operations across all centers meet PACE regulatory requirements, including documentation standards, scope of practice rules, and participant rights.
Collaborate with PACE leaders to prepare for audits, surveys, and regulatory reviews.
Identify risk trends and develop strategies to mitigate clinical risks across PACE centers
Qualifications
Education:
Bachelor’s degree in Nursing.
Master’s degree in Nursing, Healthcare Administration, or related field preferred.
License/Certifications:
Active RN license in Massachusetts (or eligibility for MA licensure).
Required:
Minimum 7–10 years of nursing experience, including at least 5 years in a nursing leadership role.
Experience in PACE model of care, geriatrics/primary care medical group, long-term care setting, home care setting, or complex care management environments.
Demonstrated success leading clinical teams, managing multi-site operations, and implementing quality improvement initiatives.
Proven communication, relationship-building, and organizational skills
Dynamic and hands-on nursing leader with strong teamwork approach and ability to foster cross-team partnerships
Preferred:
Experience in policy development, committee leadership, and standardization of nursing practice across multiple locations.
Knowledge of PACE regulatory requirements and interdisciplinary care delivery
Pay Range Disclosure:
In accordance with the Massachusetts Wage Transparency Act, the pay range for this position is $160,000 to $165,000 per year , which reflects what we reasonably and in good faith expect to pay at the time of posting. Final compensation will depend on the candidate’s experience, skills, and fit with the role’s responsibilities.
