Guided by our mission—<em>Through dedication, respect, and compassion for our patients and one another, we aim to inspire hope, and to advance the health and well-being of our patients, community, and staff</em>—we are more than a doctor’s office; we’re a trusted community partner.</p><h3><br></h3><h3>Duties &amp; Responsibilities</h3><ul><li>Provide triage for behavioral health and support crisis and emergency situations.</li><li>Deliver crisis intervention and individual counseling services; manage assigned caseload.</li><li>Serve as a “bridge” for integrated care, including warm handoffs and patient check-ins with medical providers.</li><li>Conduct mental health assessments and develop treatment plans.</li><li>Support primary care providers, nurses, and other team members with consultation.</li><li>Engage with cross-agency treatment and prevention teams in the community.</li><li>Record patient case information in the EMR within 72 hours of contact.</li><li>Maintain compliance with HIPAA, ethical standards, and NH state laws regarding crisis management and involuntary admittance.<br><br></li></ul>",
