Description

Are you looking for a career where every day has purpose? Join HealthFirst Family Care Center, where you’re not just filling a position—you’re making a meaningful impact. As New Hampshire’s 2024 Health Care Business of the Year (Business NH Magazine), we provide integrated medical and behavioral health care to all, regardless of ability to pay. Step into a role where your skills and compassion directly change lives. Crisis Specialist Who You Are You’re passionate about supporting patients in crisis and providing immediate, compassionate care. You thrive in a team-based, integrated care model where behavioral health and medical services work hand in hand. You have experience in crisis intervention, assessment, and treatment planning, and you can remain calm under pressure. You communicate with empathy and clarity, always respecting confidentiality and cultural differences. You are licensed or on a path to licensure in the State of NH and committed to ongoing professional development. Who We Are HealthFirst Family Care Center is a mission-driven, federally qualified health center serving 29 rural New Hampshire townships. We offer primary care, behavioral health, substance use disorder treatment, and community support services. Guided by our mission— Through dedication, respect, and compassion for our patients and one another, we aim to inspire hope, and to advance the health and well-being of our patients, community, and staff —we are more than a doctor’s office; we’re a trusted community partner. Duties & Responsibilities Provide triage for behavioral health and support crisis and emergency situations. Deliver crisis intervention and individual counseling services; manage assigned caseload. Serve as a “bridge” for integrated care, including warm handoffs and patient check-ins with medical providers. Conduct mental health assessments and develop treatment plans. Support primary care providers, nurses, and other team members with consultation. Engage with cross-agency treatment and prevention teams in the community. Record patient case information in the EMR within 72 hours of contact. Maintain compliance with HIPAA, ethical standards, and NH state laws regarding crisis management and involuntary admittance.
