We help individuals with developmental disabilities live their best lives, and we help our neighbors access quality health and mental health services—core to building lifelong resilience and wellbeing. Together, our interrelated programs provide a whole-person, whole-family, and whole-life approach that unlocks solutions for a lifetime. DD Division The natural supports of family and community are the most effective means to support people. In a home environment we learn basic trust and a sense of self, culture, heritage and how to live with others. It is through this experience that we develop the skills to practice the self-regulation necessary to transcend into productive adults. It is within this culture that The Foundling’s DD Division provides persons with developmental disabilities assistance to reach their maximum potential Responsibilities The DSP ll will have daily contact with the Assistant Residence Manager and/or Residence Manager and Person Supported Individuals. Residence Manager will provide supervision and individuals will receive care and supervision. Regular contact will occur with service providers in the community, as well as, school, day program and transportation staff. The DSP II will: Ensure all activities are designed to foster growth and support with a person-centered approach. Ensure appropriate supervision, health and safety of individuals; Implement Individual Plan of Protective Oversight. Advocate for individuals in the community (medical appointments, church, recreation activities etc). Ensure individual’s plan of care is implemented. Ensure that individual medication is administered as prescribed. Assist in daily living skills (e.g. personal hygiene, food preparation, housekeeping, laundry, shopping) Maintain facility in compliance with the OPWDD and COA standards. Facilitate individual’s learning and skill training in fire safety. Implementation and documentation of individual’s goals. Complete daily progress notes and other individual logs. Maintain updated certifications in CPR/first aid, SCIP, and AMAP. Provide atmosphere conducive to positive behavior. Observe, report regressive and unusual behavior and work cooperatively with clinical staff. Planning and preparing balanced meals. Cleaning and maintaining of the residential unit. Assist individuals with the purchase of groceries, clothing, hygiene supplies and other items needed. Attends appropriate workshops, trainings, and meetings. Assist in providing spiritual development. Assist in providing ethical and moral values. Transport and escort individuals on recreational activities, medical appointments, home visits, etc Assist in the development of social skills. Remain on shift until your relief has arrived. Debrief relief regarding any issues in the residence. Complete job-related tasks as needed. Essential Functions Assist with personal hygiene. Provide person with help with baths, shampoos and showers. Perform and/or assists persons with simple treatments such as: Sitz bath, binder application, remove and reapplication of Sequential Compression Device wraps, bandage application, soaking to limbs, simple dressing changes (normal saline), pressure ulcer care (normal saline, hydrocolloid dressing.), colostomy care on established colostomy patients, application of ointments and creams, including medicated topicals, as directed by the RN. Administers gastrostomy and jejunostomy feeds, including verifying feeding rates, changing bags, check for residuals, establishing patency and placement of feeding tube prior to feeding and recognizing and reporting deviation from normal. Assemble oxygen and suction set-ups Supporting people who may have one of the following High Medical Needs: multiple chronic diseases wound care colostomy care Frequent medical hospitalizations oxygen g-tube multiple/frequent medical appointments and/or multiple medications (number of meds per med pass) Alzheimer's disease and/or a diagnosis of dementia requiring daily transfer age 65+ to age in place The New York Foundling is committed to attracting and retaining a diverse employee population, the Foundling will honor your experiences, perspectives and unique identity. AMAP, SCIP, CPR, and First Aid Certifications preferred. Valid NY, PA, CT and NJ Driver’s License with 6 or less points Experience working with adults that have developmental disabilities.
