You will liaise with patients, families and all health care providers and using clinical assessment, monitoring and management skills to provide the best possible patient care. You will help patients to manage pain and symptoms and avoid unnecessary hospitalization, as well as manage acute and episodic episodes of complex disease.</p>\n<p> </p>\n<ul>\n <li>Delivers comprehensive primary health care services (including but not limited to medication reconciliations, prescribing medication, ordering lab work, chart reviews, physical exams) to at-risk adults and seniors facing access barriers to health care.</li>\n <li>Liaises, as required, with existing patient Physicians and other community partners to help address patient needs/goals. </li>\n <li>Provides health promotion and education to patients in the Specialty Programs by utilizing a diverse range of methods tailored to each patient’s needs.</li>\n <li>Educates patients on effective disease prevention and/or prevention of further disability.</li>\n <li>Conducts focused assessments based on Intake team notes, , Care Coordinators’ communication or Managers’ escalation and/or barrier identification and provides recommendations to the Care Coordination team. </li>\n <li>Provides consultation and support to Ontario Health atHome Champlain Care Coordinators and key stakeholder partners. </li>\n <li>Ensures best practice by bringing forward ideas and opportunities to Specialty Program leaders for dialogue and consideration.</li>\n <li>Assists with the collaborative development and implementation of Ontario Health atHome Champlain policies and procedures as well as joint initiatives with community partners</li>\n <li>Acts as a Specialized Program advocate as required, and ensures positive public relations and effective co-ordination of services through ongoing liaison and participation on internal and external committees.</li>\n <li>Participates in regular professional development activities such as academic rounds and other educational events.</li>\n</ul>\n<p><strong>Team/Department: </strong>Regional Palliative Consultation Team<strong></strong></p>\n<p><strong>Hours of work: </strong>Monday to Friday – 8:30am to 4:30pm (35 hours/week) with 24/7 on-call duties on a rotation</p>\n<p><strong>FTE:</strong> 1.0</p>\n<p><strong>Starting Salary: </strong>$59.855 - $69.523</p>\n<p><strong>Affiliation: </strong>ONA</p>\n<p><strong>Length of temporary assignment:</strong> June 11, 2027</p>\n<p><strong>Reporting to: </strong>Manager, Patient Services</p>\n<p><strong>Office Location: </strong>Labelle</p>\n<p> </p>\n<p><strong>What must you have?</strong></p>\n<ul>\n <li>Membership, in good standing, with the College of Nurses of Ontario</li>\n <li>Registered Nurse in the Extended Class (RN EC designation) </li>\n <li>Working knowledge of community resources and roles of health care professionals</li>\n <li>Reviews the discharge care plan and confirms outstanding medical tests have been scheduled and transportation etc. is available</li>\n <li>Conducts medication reconciliation and reviews medication protocol with patients and caregivers and answers any questions</li>\n <li>Initiates contact with primary care physician and provides update on patient acute care event and post-discharge regime. Facilitates, as appropriate, a one-week patient follow-up visit with the primary care physician</li>\n <li>Identifies patients requiring an accelerated assessment and home care services and works with the Care Coordinator to facilitate the home assessment visit. In some situations, RRNs may complete an in-hospital assessment and facilitate services as required</li>\n <li>Informs and supports the Care Coordinator in developing the patient’s care plan and ensuring a smooth transfer of the primary care physician and pharmacist to the ongoing care team</li>\n <li>Answers any questions or concerns of the patient and caregiver </li>\n <li>Act as a spokesperson as required, and interpret the role of OH atHome to patients, health care professionals and to the public.  Ensures positive public relations and effective co-ordination of services through ongoing liaison and participation on internal and external committees.</li>\n <li>Assesses for and promotes a safe environment for patients, caregivers, family members, and staff. Adheres to health and safety policies/ practices developed and implemented by OH atHome</li>\n <li>Participates in establishing, maintaining, and monitoring standards for the Case Management.
